Since June 28, 2011, counsel who, for a fee or other retribution, represent, advise or consult with a person who is the subject of a proceeding must provide the
IRB with the name of the organization of which he or she is a member in good standing, and with his or her membership identification number. Counsel can fill out the
Counsel Contact Information form (

KB) form, print it and return it to the
IRB.
